Job summary

ProCare Therapy in Jefferson, MA is seeking a licensed School-based Physical Therapist for a full-time contract serving elementary students in a collaborative special education setting. You will provide school-based PT services, participate in IEP goals, evaluations, and collaboration with educators and families.

Start date is August 24, 2026 with end date June 20, 2027; onsite only, 37.5 hours per week, five days per week.

Qualifications

• Active Massachusetts Physical Therapist license required.
• Experience in school-based or pediatric physical therapy preferred.
• Ability to collaborate with educators, families, and related service providers.

Responsibilities

• Provide school-based physical therapy services to elementary students.
• Complete evaluations and re-evaluations to determine eligibility and service needs.
• Develop, implement, and monitor IEP goals related to physical therapy services.
• Collaborate with teachers, families, administrators, and multidisciplinary team members.
• Maintain documentation and Medicaid billing as applicable.
• Participate in IEP meetings and contribute recommendations.
• Monitor student progress and adjust treatment plans as needed.
