Job summary

Gulf Coast JFCS in Florida seeks an Elder Services Case Manager to oversee caseloads for elder clients, coordinating services and developing care plans. You will screen and assess needs, determine Medicaid eligibility where applicable, and maintain regular contact with clients while ensuring proper documentation and reporting.

Qualifications

• Bachelor’s degree in social work or related field required.
• Two years of case management experience preferred.
• Knowledge of community resources and services.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office; driving is required with insurance.
• Ability to lift and carry case files up to 25 lbs.

Responsibilities

• Provide caseload management for elder services according to funder guidelines.
• Screen, assess, and develop care plans addressing service needs.
• Evaluate client eligibility for programs including Medicaid.
• Maintain regular client contact per program guidelines.
• Coordinate information and referrals for Medicare or Hospice.
• Document accurately and timely, maintaining client records.
• Report abuse or neglect to Florida Abuse Hotline as required.
• Meet productivity targets related to caseloads.
• Perform other duties as assigned.
