Job summary

UP Health System - Portage is seeking an Elder Associate (CNA) to join our team. This PRN role supports RN staff with essential patient care and daily living activities in a long-term care setting.

Qualified candidates will have a Michigan CNA certification within 4 months of hire, BLS certification, and a high school diploma. 1–2 years CNA experience is preferred; ServSafe is required within 6 months of hire.

Qualifications

• Michigan CNA certification must be obtained within 4 months of hire.
• Basic Life Support (BLS) is required (provided upon hire).
• ServSafe certification required within 6 months of hire.
• High School Diploma or equivalent required.
• Graduate of an approved nursing/disciplined program.

Responsibilities

• Provide basic patient care under RN supervision.
• Demonstrate safe patient handling and activities of daily living support.
• Assist with bed mobility, turning, baths, and linen changes.
• Take and document vitals and input/output accurately; document timely.
• Respond to Elder call lights and paging system promptly.

How you'll contribute

An Elder Associate who excels at this role:

• Perform basic patient care skills under the direction of a Registered Nurse.
• Demonstrate safe practices with caring for patients.
• Provides general nursing care such as position the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, bed bath, oral and hair care, changing bed linens, cleaning and straightening patient room and other general care necessary.
• Takes and records vitals and intake and output measurements accurately. Documents in a timely manner.
• Answers Elder call lights, telephone, and paging system and respond as appropriate.

What we're looking for

• Applicants must have a Michigan Nurse Aide certification (CNA) required (must be obtained within 4 months of hire) Additional Requirements include:

• Graduate of a Program in Discipline required

Portage Pointe is a long-term care facility with 60 beds to offer to the senior community and is known as the premier nursing home serving the entire Upper Peninsula.
