Job summary

Valleywise Health is seeking a Healthcare Documentation Integrity Specialist to ensure the integrity of the EHR by monitoring, measuring, and reporting on clinician documentation. You will compile and relay feedback on errors to prevent recurrence and support quality assurance across the HIM team.

The role involves coordinating with HDI Supervisors, Coders, and Clinical Documentation Improvement Specialists, performing QA reviews, and assisting with training.

Qualifications

• Requires an associate degree in nursing or health information technology or related field, or equivalent training and experience.
• A bachelor’s degree in nursing, health information administration, or related field is preferred.
• Two (2) years of progressively responsible experience in Health Information Management, Clinical Documentation Improvement, Coding, or Clinical Auditing/Review related to Revenue Cycle.

Responsibilities

• Ensure integrity of the Electronic Health Record by monitoring, measuring, and reporting on documentation created by healthcare providers.
• Compile feedback for healthcare providers on critical and major errors to prevent recurrence and support quality assurance.
• Coordinate workflow and liaise with Clinical Documentation Improvement Specialists and Coders.
• Perform quality reviews of patient information and data entry for provider notifications and tracking of incomplete health records.
• Assist with training/mentoring within the HIM Department as needed.
• Identify patterns and provide feedback to improve EHR documentation integrity.
