Job summary

Trinity Health Mid-Atlantic is hiring a Part-time Pharmacist for Mercy Fitzgerald Hospital’s Emergency Department in Darby, PA. This role supports an expanded emergency pharmacy program and includes bedside medication histories, counseling, and rapid response during emergencies.

Shifts are 12-hour nights (8p–8a) with 24 hours/week; per diem coverage for decentralized and centralized distribution. Requires Pharm.D., PA licensure, and ASHP PGY-1 or 3 years hospital experience.

Qualifications

• Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
• Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
• Registered pharmacist in Pennsylvania required.
• Preferred: Experience with Pyxis MedStation, EPIC
• BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)

Responsibilities

• Assess patient data to identify medication-related problems and updates in the medication history.
• Participate in emergencies and formal alerts (trauma, stroke, sepsis, STEMI) as required.
• Review and interpret orders to ensure appropriate drug choice and dosing for age, weight, and comorbidities.
• Consult with providers, nurses and others on pharmacotherapy optimization.
• Provide timely drug information to healthcare professionals and patients.
• Document patient care activities in the EMR per guidelines.

Position Purpose

We are hiring Pharmacists to support operations and our expanded emergency pharmacy program! We are expanding our shift coverage to include a decentralized pharmacist. We are searching for per diem help to cover this role as well as centralized distribution. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.

As a Pharmacist, you will:

• Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
• Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
• Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients.
• As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
• Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
• Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines.

Minimum Qualifications:
