Job summary

Central Neighborhood Health Foundation is seeking a Clinic Liaison to coordinate care for patients discharged from hospital ED or inpatient settings. You will work with patients, caregivers, and hospital teams to ensure access to a medical home and follow-up care, reducing barriers to services.

Responsibilities include coordinating with nurses and discharge planners, facilitating appointments, obtaining records, and linking patients to resources. Travel to CNHF sites may be required.

• Experience managing patient issues and coordinating care.

• Must have a valid California Driver’s License.

Responsibilities

• Liaise between CNHF and Emergency Department/returned patients.
• Coordinate care access and work with care teams to address patient needs.
• Collaborate with hospital nurses, care coordinators, and discharge planners.
• Assist patients in transitioning to a medical home and arranging follow-up care.
• Explain available services and connect patients to resources.
• Obtain discharge paperwork and relevant hospital records as available.
• Identify barriers to care and link patients to community resources.
• Outreach to discharged patients to ensure appointments are kept; attend meetings.
• Travel to other CNHF sites as needed; perform other duties as assigned.
