About This Role

The ED Care Coordinator will support patients in the Emergency Department by identifying high-risk individuals and connecting them to necessary community resources. This role is ideal for someone with strong communication skills and a passion for patient advocacy.

• Schedule: Full‑time, Day shift (12‑hour shifts, Rotating)

What You’ll Do

• Identify vulnerable patients at high risk for readmission or high ED utilizers.
• Collaborate with patients, ED staff, and community providers to develop comprehensive care plans.
• Act as a patient advocate by connecting patients to appropriate community resources.
• Maintain and update a directory of available services by geographical area.
• Consult with others to identify potential community resources for resolving health, psychosocial, or financial barriers.
• Collaborate with the behavioral health team to connect patients to mental health programs.
• Interview and assess patients/family members to formulate care plans prior to unit transfer.
• Create CRISP Care Alerts for complex and high-utilizing patients in collaboration with the ED team.

Requirements

Education:

• Associate of Science degree required, Bachelor’s degree preferred

Experience:

• 1 – 3 years’ experience in an acute care setting, with at least 1 year as a case manager, discharge planner, or care coordinator preferred

Licenses & Certifications:

• Current Maryland RN License
• BLS certification required
