Libertana seeks an ECM/CICM Program Supervisor to oversee day-to-day ECM and CICM operations in a field/remote setting in Kentucky. Lead staff, coordinate case management services, and ensure adherence to contractual, regulatory, and organizational standards.

You will coach, supervise, and develop care management teams, manage workflow, and drive high-quality, member-centered care while aligning with Medi-Cal and managed care requirements.

Qualifications

  • Bachelor's degree in Social Work, Nursing, Healthcare Administration, Human Services, Public Health, or related field required.
  • Master's degree preferred.
  • Minimum 3 years of experience in case management, care management, social services, population health, or healthcare operations.
  • Minimum 1-2 years of supervisory or team leadership experience.
  • Experience working with Medi-Cal populations and complex care management programs.
  • Experience with ECM, CalAIM, CICM, care coordination, or managed care programs preferred.

Responsibilities

  • Directly supervise Care Managers, Case Managers, Community Health Workers, and other assigned staff.
  • Conduct regular one-on-one supervision, team meetings, and case consultations.
  • Provide coaching, mentoring, and professional development opportunities.
  • Participate in recruitment, orientation, training, and onboarding of new staff.
  • Monitor staff productivity, performance goals, and quality metrics.
  • Conduct performance evaluations and assist with corrective action plans when necessary.
  • Oversee daily ECM/CICM operations and workflow management.
  • Ensure appropriate assignment and distribution of members and caseloads.
  • Monitor member engagement, outreach activities, and service delivery.
  • Identify operational barriers and implement process improvements.
  • Provide guidance regarding complex member needs and high-risk cases.
  • Ensure timely completion of assessments, care plans, care coordination activities, and documentation.
  • Review high-risk, escalated, or complex cases and participate in interdisciplinary case conferences.
  • Promote person-centered care planning and whole-person care principles.
  • Facilitate collaboration with medical, behavioral health, social service, community-based, and long-term care providers.
  • Conduct chart reviews and documentation audits.
  • Monitor compliance with ECM and CICM program requirements, health plan expectations, and DHCS regulations.
  • Ensure completion of required assessments, care plans, encounters, and member contacts.
  • Track quality indicators and corrective action activities.
  • Assist with internal and external audits and quality improvement initiatives.
  • Monitor productivity and utilization metrics.
  • Support implementation of performance improvement plans.
  • Maintain effective working relationships with managed care plans and community partners.
  • Participate in health plan meetings, case conferences, and collaborative initiatives.
  • Performs other duties as assigned.

POSITION SUMMARY:

The ECM/CICM Program Supervisor is responsible for day-to-day oversight, coordination, and supervision of Enhanced Care Management (ECM) and Community Integrated Care Management (CICM) staff and operations. This position ensures the delivery of high-quality, person-centered care management services that meet contractual, regulatory, and organizational requirements.

The Supervisor provides leadership, coaching, and performance oversight to care management staff while promoting positive health outcomes, member engagement, interdisciplinary collaboration, and compliance with Medi-Cal, managed care plan, and organizational standards.

PHYSICAL REQUIREMENTS:

  • Stand, sit, talk, hear, and use of hands and fingers to operate computer, telephone, and keyboard on a frequent basis up to 20% of the time.
  • Reach, stoop, kneel and bend up to 15% of the time
  • Moderate amount of walking up to 15% of the time.
  • Moderate amount of driving up to 50%of the time.
  • Close vision requirements due to computer work on a frequent basis
  • Light to moderate lifting may be required up to 25lbs on a frequent basis.
