Job summary

CHS Middletown - Middletown, PA is seeking an Evaluation & Education Support Coordinator to collaborate with teachers and interventionists to meet the developmental needs of children across physical, language, social-emotional and cognitive domains.

You will lead the early intervention/evaluation process, support staff in implementing supports, and coordinate data collection to guide referrals and interventions. A Master’s degree is preferred and US work authorization is required.

Qualifications

• Bachelor's degree in early childhood education, special education, early intervention, child development, or related field; a related Master's is preferred.
• 5+ years of experience in early intervention.
• Experience interpreting IFSPs and IEPs preferred.
• Strong organizational and time-management abilities.
• Knowledge of speech, occupational, and physical therapy procedures.
• Ability to adapt in a fast-paced environment with shifting priorities.
• Demonstrated integrity and commitment to CHS/MHS mission.
• U.S. work authorization and ability to pass pre-employment checks.

Responsibilities

• Coordinate screening and evaluation schedules with teachers and families to support individual child needs.
• Assist in evaluating behavioral concerns and disseminate intervention resources to staff.
• Record observations and document behaviors with teachers for progress reports.
• Research and recommend tools to help children access curriculum with delays or disabilities.
• Obtain medical records in partnership with the Center Nurse for each child under evaluation.
• Refer children to appropriate programs based on IFSP/IEP goals and assist with related forms.
• Oversee speech, occupational, and physical therapy processes as needed.
• Organize schedules of early intervention providers visiting the Center and coordinate transport when required.
• Connect families with additional services when early intervention isn't indicated.
• Provide monthly data reports to the Health Services Manager.
• Lead Child Study Team meetings and record child-specific support plans.
• Plan training for teachers and parents to support children with special needs.
