Responsibilities

• Provides culturally sensitive home visitation services for families that promote positive parent-child interaction, child developmental skills, and healthy and safety practices through curriculum and other educational materials.
• Links families to medical providers and community resources to support health and development.
• Administers developmental assessments and screenings to monitor progress and maintain timelines.
• Works with families to develop individualized family service plans and goals.
• Maintains accurate electronic case files and program records.
