Job summary

Easterseals NH is seeking a Social Worker to join our Early Childhood Programs in Manchester. You will provide assessments, treatment planning, and therapeutic services for children and families facing mental health and socio-emotional challenges.

In this role, you will collaborate with multidisciplinary teams, deliver crisis intervention when needed, and maintain thorough documentation in EMR systems, supporting a holistic child- and family-centered model.

Qualifications

• Master’s or Doctoral degree in Social Work or related field.
• Licensed as LCSW or LICSW required.
• 2+ years in community mental health with crisis intervention.
• Experience with EMR systems.
• Experience in Trauma-Informed Care environment.

Responsibilities

• Conduct thorough psychological assessments and evaluations in alignment with program, agency, and licensing standards.
• Develop, implement, and maintain individualized treatment plans.
• Provide therapeutic treatment to clients in individual and group therapy sessions in various clinical and/or natural settings.
• Complete and maintain accurate and timely clinical documentation that aligns with program standards.
• Coordinate care needs and case management with internal & external healthcare providers, support staff, and early childhood professionals to ensure high-quality services.
• Educate clients and families about mental health conditions and treatments.
• Offer crisis intervention services when necessary.
• Develop progress reports and reporting for state/federal requirements.
• Participate in staff training and contribute to integrated services within Early Childhood Centers of Excellence.
