Job summary

Next Door Milwaukee is seeking a Health Coordinator to ensure health screenings, follow-up care, and documentation across Early Head Start and Head Start programs. The role involves coordinating records, referrals, and family support to help children stay healthy and ready to learn.

The ideal candidate will manage health data in ChildPlus, collaborate with Family Advocates and educators, and maintain compliant health documentation while supporting program quality and coordination across sites.

Qualifications

• Bachelor’s degree in public health, nursing, health education, social work, or related field.
• At least two years of experience in community health or early childhood health.
• Experience with diverse communities.
• Strong organizational and multitasking skills.
• Ability to work collaboratively with families and staff.
• Attention to detail and documentation.
• Experience with data entry and analysis in a database or case-management system.
• Strong written, verbal, and interpersonal communication skills.
• Knowledge of Head Start standards preferred.
• Valid driver’s license and transportation.

Responsibilities

• Coordinate screenings and follow-up care, ensuring health requirements are completed on schedule.
• Develop systems to identify upcoming or missing health requirements and track timelines.
• Coordinate health events, enrollment clinics, health fairs, and related activities.
• Obtain health records, consents, and assessments from families and providers.
• Coordinate referrals and follow-up services when concerns arise.
• Ensure children with chronic conditions have appropriate health plans and staff guidance.
• Document medication administration, allergies, and dietary accommodations.
• Enter and maintain health information in ChildPlus and other required systems.
• Conduct regular audits of health records for accuracy and completeness.
• Provide training and guidance to staff on health requirements and documentation.
• Maintain confidentiality of information.
