Job summary

Next Door in Milwaukee seeks a Health Coordinator to ensure that every child enrolled in our Early Head Start and Head Start programs receives required health screenings and follow‑up care so they are healthy and ready to learn.

You will enter and maintain health data in ChildPlus, coordinate screenings and referrals, and help families overcome barriers while ensuring documentation is accurate across all Next Door programs and sites.

Qualifications

• Bachelor's degree in public health, nursing, health education, social work, or related field.
• At least two years of experience in community health or health services coordination.
• Experience with diverse families and strong documentation skills.
• Knowledge of Head Start health requirements and data systems preferred.

Responsibilities

• Monitor each child's health records to ensure required screenings are completed on time.
• Develop a system to identify upcoming or overdue health requirements.
• Coordinate health events, enroll clinics, health fairs, and related activities.
• Obtain health records, screening results, consents, and assessments from families and providers.
• Coordinate referrals and follow‑up services for identified health concerns.
• Support Individualized Health Plans and staff training on implementation.
