Qualifications

Responsibilities

• Assess: Performs comprehensive assessment of physical, social, and psychological status and information from the care team.
• Plan: Develops individualized care plans with patient, family, and team involvement.
• Implement: Coordinates and delegates care to the interdisciplinary team to meet plan of care.
• Evaluate: Reviews outcomes and revises care plans as needed.
• Professionalism: Promotes nursing profession and supports the development of others.
