Job summary

Qualifications

Responsibilities

• Evaluates patients and develops care plans with modalities and exercises.
• Gathers data from medical records and families for documentation.
• Sets functional goals and provides evidence-based interventions.
• Documents evaluations, tests, progress, and charges.
• Communicates progress to physicians and coordinates scheduling and discharge planning.
• Supervises PTA and Rehab Tech as required.
