Job summary

Wolcott Hall Nursing Center in Torrington, CT is seeking a Physical Therapist to evaluate patients and create individualized rehab plans that maximize movement and independence.

The PT will collaborate with an interdisciplinary team, review referrals, implement treatment plans, and educate patients and families on home exercise programs. A current PT license and at least one year of PT experience are required.

Qualifications

• Graduate of an accredited PT program
• Current PT license, in good standing with the State licensing board
• Minimum of one year PT experience in a corresponding setting
• Federal-, State- and GHR-required credentials current and on file

Responsibilities

• Plans and administers medically prescribed physical therapy treatment to restore function and relieve pain
• Reviews physician's referral and patient condition to determine PT treatment
• Tests and measures strength, movement, and functional capacity to develop or revise treatment programs
• Instructs patient and family in treatment procedures to be continued at home
• Confers with physician and other practitioners to optimize care and integrate PT with other health services
• Orients and directs work activities of therapy assistants and aides
