Qualifications

• Associates degree in Physical Therapy required.
• Active license/permit to practice in Texas.
• CPR certification required while employed.
• Must have a valid driver’s license and be able to travel to locations.
• Ability to implement care plans and communicate findings to therapist/physician.
• Ability to educate patients and families on home safety and medical equipment.

Responsibilities

• Provide patient care under supervision of a licensed Physical Therapist.
• Document treatment according to physician orders and policies.
• Educate patients and families and counsel regarding treatment plans.
• Maintain patient status updates with supervising therapist and team.
• Document daily notes and complete required forms timely.
• Meet productivity standards and participate in meetings.
