Job summary

Qualifications

• Minimum 1–3 years of relevant experience.
• Associate's degree required or equivalent.
• Current NYS license as PTA required.
• Limited permit for outpatient settings acceptable.

Responsibilities

• Administers direct patient care per physician prescription.
• Submits required documentation per department policies.
• Maintains progress notes and discharge summaries.
• Responds to department emergency procedures for patient safety.
• Participates in quality improvement chart reviews.
• May attend family meetings pertaining to patient welfare.
• Performs related duties as required; ADA note included.
