Job summary

Health Here is seeking a Physical Therapist to provide comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary environment.

The successful candidate will conduct thorough patient assessments, develop personalized plans, educate patients, maintain documentation, and collaborate with the care team to ensure holistic patient care.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, valid state PT licensure or certification.
• Preferred CPR certification.
• 0-1 years of experience.

Responsibilities

• Conduct thorough patient assessments to evaluate physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans utilizing therapeutic exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, and body mechanics for effective management of conditions.
• Maintain accurate documentation of patient progress, treatment plans, and outcomes.
• Collaborate with team members to ensure holistic patient care.
• Perform additional duties as assigned.

Education
