Job summary

The Physical Therapist (PT) at Synchrony provides a full range of physical therapy services including assessment, treatment planning, and interventions in an interdisciplinary setting in Kendallville, Indiana.

The role emphasizes patient-centered care, documentation, and collaboration with the team to restore function and quality of life. A degree in physical therapy, state licensure, and 0–1 years of experience are expected, with CPR certification preferred.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Zero to one year of experience in PT preferred.
• Active state PT licensure or certification; CPR certification preferred.

Responsibilities

• Perform comprehensive assessments to evaluate patients' conditions and rehab needs.
• Develop and implement individualized treatment plans and interventions.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate progress notes and documentation per policy.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Assist with other duties as assigned.
