Qualifications

• Requires Doctorate or degree in Physical Therapy and Illinois PT license.
• BLS certification within 30 days of hire.

Responsibilities

• Assist patient transfers with safe body mechanics.
• Provide patient education to patients and families.
• Develop and adjust care plans with goals and discharge plans.
• Educate patients and caregivers using effective methods.
• Evaluate findings to determine PT diagnoses and outcomes.
• Perform examinations with appropriate methods.
• Document care per policy CTSCP67.
• Screen patients to determine therapy needs.
• Supervise support personnel and provide feedback.
