Qualifications

• Graduate of an accredited physical therapy curriculum with a Bachelor, Master, or Doctoral degree and PT certification.
• California Physical Therapist License required at hire.
• National Provider Identifier required at hire.
• Basic Life Support (BLS) required.

Responsibilities

• Deliver direct patient care in alignment with the Physical Therapy Practice Act, licensing bodies, and Kaiser Permanente HealthPlan.
• Provide accurate and timely documentation of services per state and federal requirements and Kaiser policies.
• Participate in multidisciplinary clinics, patient care rounds, and case management conferences as requested by management.
• Refer patients to appropriate medical providers when care is outside of scope of practice.
• Instruct patient education classes and/or groups as requested by management.
• Provide department in-service and orientation training as needed.
• Mentor staff and serve as a clinical instructor as requested by management.
• Participate in performance improvement projects or committees as requested by management.
