Job summary

Concentra in Houston is seeking a qualified Physical Therapist to deliver comprehensive rehabilitative services for injured and diseased patients. You will evaluate, design, implement, and monitor therapy programs, while collaborating with physicians and staff to ensure high quality care.

The role emphasizes documentation accuracy, patient education, and compliance with HIPAA and center policies, contributing to optimal patient outcomes and service excellence.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Masters’ degree in Physical Therapy is preferred.
• Ortho-outpatient experience is preferred.

Responsibilities

• Deliver high quality patient care.
• Perform evaluation of muscle, skeletal, neurological and other physical problems.
• Coordinate with multidisciplinary teams for treatment.
• Document patient care thoroughly and accurately.
• Follow quality assurance policies and regulatory requirements.
• Communicate effectively with staff, physicians and patients to ensure coordinated care.
• Plan and implement physical therapy treatment programs.
• Educate patients on exercises and proper use of supports.
• Administer modalities, therapeutic exercises, gait training and hydrotherapy.
• Assist with marketing tours as needed.
• Ensure compliance with HIPAA and privacy regulations.
• Participate in professional development and training.
