Qualifications

• Bachelor's degree (or greater) in Physical Therapy required.
• Active, unencumbered license in the state of employment required.
• Basic computer skills and ability to navigate EHR and service delivery software.

Responsibilities

• Return patient to optimal physical function in SNF setting.
• Instruct patients on maintenance after discharge and independent exercise.
• Collaborate with physicians and therapy team to achieve goals.
