Job summary

Autumn Lake Healthcare at Glen Hill in Danbury, Connecticut is seeking a licensed Physical Therapist (PT) to join our rehabilitation team. You will evaluate patients, design treatment plans, and help improve mobility and independence in a holistic clinical setting.

You will work closely with doctors, nurses, and families to customize care plans and track progress, delivering hands-on therapy and education to patients and caregivers in a state-of-the-art facility.

Responsibilities

• Conduct thorough assessments and evaluations of patients' physical function, mobility, and independence
• Develop and implement individualized treatment plans that address patients' unique needs and goals
• Provide direct patient care, including exercises, stretches, and other therapeutic interventions
• Collaborate with patients, families, and other healthcare professionals to ensure effective communication and coordination of care
• Maintain accurate and detailed records of patient progress and treatment outcomes
• Participate in interdisciplinary team meetings to discuss patient progress and develop comprehensive care plans
