Qualifications

• Must hold a Physical Therapist license.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care; develop care plans adhering to standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Assist with regular standing/walking and lifting/transferring of patients and equipment as needed.
• Administer procedures as part of the planned rehabilitation including manual techniques, ambulation, exercises, modalities, and wound care.
• Provide counseling to patients and families regarding physical disorders.
• Generate discharge summaries ensuring regulatory compliance.
