You will collaborate with physicians and staff to deliver evidence-based therapy, evaluate patients’ progress, and tailor treatment plans to maximize functional outcomes.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed Physical Therapist in the state.
• CPR Certification.

Responsibilities

• Perform physical therapy assessments of referred patients.
• Provide direct patient care to evaluate functional abilities.
• Collaborate with the care team to develop treatment plans.
• Document treatment sessions in PCC EMR.
• Participate in onboarding and ongoing professional development.
