Responsibilities

• Gather data from chart and care team; discuss with physician as needed.
• Independently assess all patient types as assigned.
• Recognize additional needs during evaluation and request/perform tests as appropriate.
• Interpret results and set relevant, measurable goals.
• Reassess patient progress per policy and when needed.
• Identify equipment needs and discuss with patient and family.
