Qualifications

• NEW GRADS ENCOURAGED TO APPLY.
• Must have a valid professional license by discipline for state.
• Prefer at least one year experience in LTC.
• Exceptional patient service skills and clinical excellence.
• Passionate about post-acute rehab and older adults.
• Ability to relate well with residents, families and staff.
• Must pass post offer background check and drug screen.

Responsibilities

• Provide individual therapy services per care plans.
• Assist in resident assessments as needed.
• Develop therapy plans with MDS input.
• Adjust plans to support care goals.
• Prepare equipment and brief residents on procedures.
• Participate in discharge planning and care plan development.
• Interview residents and family as necessary.
• Coordinate therapy appointments with other departments.
• Follow safety standards in therapy.
• Maintain supplies and keep therapy area orderly.
