Job summary

Responsibilities include maintaining productivity, ensuring evidence-based documentation, and preparing patients for discharge when maximum benefit is reached. Licensure in Florida and BLS are required.

Qualifications

• Bachelor's Degree in Physical Therapy with Florida licensure required.
• AHA BLS certification required and maintained.
• Level II fingerprint background and good moral character affidavit within 90 days.

Responsibilities

• Participates in interdisciplinary meetings pertaining to patient care.
• Maintains productivity standards set by leaders.
• Records daily patient documentation, statistics, and charges timely.
• Ensures documentation reflects evidence-based practices.
• Improves access to care for patients served.
• Conducts evaluations and provides recommendations based on findings.
• Maintains communication with patient, caregivers, and professionals.
• Develops a plan of care with deficits, activities, and measurable goals.
• Delivers treatments based on medical and functional findings.
• Prepares patient/family for discharge when therapy benefits are reached.
• Follows TRAAC and discharge prep at evaluation time.
• Provides proof of at least 1 in-service and 1 peer review.
• Maintains ongoing CEUs to sustain licensure.
