Job summary

Qualifications

• Must hold California PT license.
• 1–2 years acute care experience; home health preferred.
• Strong communication and organizational skills.

Responsibilities

• Provides direct planned patient therapy services under physician orders and according to plan of care.
• Performs initial and ongoing assessments to determine level of functioning.
• Develops and revises the plan of care in consultation with the physician and team.
• Provides patient, family and caregiver education on treatment and plan of care.
• Documents accurately and timely in accordance with policy; supervises therapy staff as needed.
• Participates in team meetings and ensures HIPAA confidentiality.
