Job summary

Atrium Health is seeking a Physical Therapist to provide in-home care within Shelby, NC. You will assess patients, develop and monitor home exercise programs, and collaborate with families to achieve goals outlined in the plan of care.

You will work with a multidisciplinary team, document outcomes, and ensure practice follows state regulations and agency guidelines while delivering compassionate, patient-centered care.

Qualifications

• Licensed Physical Therapist in North Carolina.
• Strong clinical assessment and documentation skills.
• Ability to develop home exercise programs.
• Effective communication with patients and families.

Responsibilities

• Manage an assigned patient caseload with timely care and documentation as prescribed.
• Collect health data from home assessments, telephonic contacts, and EMR.
• Implement patient care programs, modalities, and education per plan of care.
• Document and execute a care plan including discharge planning.
• Demonstrate clinical reasoning and evaluate therapy effectiveness.
• Provide patient and family education on deficits, progress, and home programs.
• Maintain communication with multidisciplinary team and physician orders.
• Align interventions with plan of care goals.
• Adhere to state PT practice acts and Home Health guidelines.
• Promote governance and research to improve outcomes.
