Job summary

Continuum Healthcare Inc. seeks a Certified Occupational Therapist Assistant (COTA/OTA) to join our rehabilitation team.

The position supports the plan of care for residents, works under supervision of a licensed therapist, and collaborates with an interdisciplinary team to deliver high-quality care in a skilled setting. Qualifications include an associate’s degree in physical or occupational therapy and active licensure.

Qualifications

• Graduate with an associate degree in physical or occupational therapy.
• Licensed or equivalent certification accepted by the designated professional accrediting body.
• Active state licensure in the employed state (Physical Therapy Assistant or Certified Occupational Therapist Assistant).

Responsibilities

• Assist and support the plan of treatment for each resident.
• Act as an active participant in the interdisciplinary team.
• Supervise and coordinate with aides per plan of care and guidelines.
• Communicate resident status and responses with supervising therapist.
• Complete documentation as directed and provide feedback to supervisors.
• Participate in center processes and in-service training as requested.
