Job summary

The role supports the plan of treatment for residents and works under the supervision of a licensed therapist within an interdisciplinary team. Ideal candidates hold an associate degree in physical or occupational therapy, are licensed or certified in the state, and have experience in older adult rehabilitation.

Qualifications

• Graduate with an associate degree in physical therapy or occupational therapy.
• Licensed/certified in the designated state as required by regulation.
• Experience in older adult rehabilitation preferred.
• Licensure in the respective discipline is required.

Responsibilities

• Provide resident rehabilitation care per plan of care.
• Collaborate with supervising therapist to monitor resident progress.
• Document care and communications accurately.
• Provide feedback to the therapist and Rehabilitation Director on clinical matters.
• Participate in center processes and in-service training programs.
• Demonstrate effective management of treatment-related activities.
• Complete required documentation to meet regulatory standards.
