EDUCATION:

Bachelor’s degree in social work, clinical psychology, or counseling from an accredited university.

CERTIFICATION/LICENSURE:

Bachelor's or Master’s degree in social work or a related field from an accredited university or college. Ability to be credentialed as a QMHP.

EXPERIENCE:

One year's experience working with individuals with chronic mental illness, intellectual disability and substance use disorders. Two years’ experience in a human services agency and knowledge about psychiatric disorders, intellectual disabilities, and substance use disorders.

SUMMARY OF RESPONSIBILITY:

Case Coordinatorto provide on-going assessment, person centered planning, coordination of care,linkage of agency and inter-agency resources and advocacy services. The CaseCoordinator is the primary provider when an individual needs multiple services.The Case Coordinator is responsible for facilitating the Person-CenteredPlanning process (PCP) so that regulatory/Medicaid compliance is assured, andthe PCP reflects consumer strengths, desires, dreams, needs and goals. The CaseCoordinator advocates for individual needs by linking, advocating, andmonitoring the utilization of community-based services and resources. The CaseCoordinator is a member of the Adult Mental Health Case Coordination team andreports to the Supervisor of Adult Mental Health Case Coordination.

RANGE OF PAY:

20.21 to 25.87 per hour
