Job summary

Center for Human Development (CHD) is seeking a Dual Licensed Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP) for our Orange, MA outpatient clinic. You will provide integrated primary care and behavioral health services across the lifespan, collaborating with a multidisciplinary team.

Qualifications

• Active and unrestricted FNP and PMHNP licenses.
• Current DEA registration and prescriptive authority in MA.
• Minimum 3 years in primary care or behavioral health.
• Proficiency with EHR systems and telehealth.
• Commitment to serving underserved populations in team-based care.

Responsibilities

• Deliver comprehensive primary care from pediatrics to geriatrics.
• Provide psychiatric evaluations and medication management.
• Collaborate with multidisciplinary teams.
• Participate in care coordination and quality initiatives.
• Maintain timely documentation in EHR.
• Support crisis intervention when needed.
• Engage in community outreach to promote health equity.
