Job summary

Center for Human Development (CHD) seeks a Dual Licensed Family Nurse Practitioner (FNP) / Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide integrated primary care and behavioral health services at the Orange, MA site. You will care for patients across the lifespan in a community-based setting with a focus on whole-person care.

Qualifications

• Active and unrestricted FNP and PMHNP licensure.
• Current DEA registration and prescriptive authority in MA.
• Minimum 3 years clinical experience in primary care and/or behavioral health.
• Proficiency in using electronic health records and telehealth platforms.
• Commitment to serving underserved populations and team-based care.

Responsibilities

• Deliver comprehensive primary care from pediatrics to geriatrics.
• Provide psychiatric evaluations, medication management, and therapy.
• Collaborate with interdisciplinary teams and community partners.
• Participate in care coordination and quality improvement initiatives.
• Maintain accurate and timely documentation in the EHR.
• Support crisis intervention and community outreach.
• Promote health equity and access in the community.
