Qualifications

Responsibilities

  • Follow NADSP code of ethics and commitments to person-centered planning.
  • Support problem-solving skills and promote autonomy.
  • Assist in habilitative service plans and ISP meetings.
  • Develop vocational skills and explore career options for individuals.
  • Identify internships and volunteer opportunities with community partners.
  • Complete documentation in Therap within required timeframes.
  • Provide transportation and navigate community resources as needed.
  • Communicate with departments to ensure service delivery.

POSITION RESPONSIBILITIES

1. Understands and follows the National Alliance of Direct Support Professionals (NADSP) code of ethics.

2. Maintains their first allegiance to the people they work for by demonstrating respect and supporting problem solving skills.

3. Has solid knowledge and understanding of person-centered planning and service delivery.

4. Promote daily thinking and problem-solving skills of the person served, using active listening and coaching skills in order to reduce their dependency on staff and increase their self-esteem.

5. Encourages growth and recognizes autonomy while being attentive and energetic in always reducing the risk of harm by providing all necessary support indicated in the IPOP or Health and Safety.

6. Assists in the development (up to and including attending ISP meetings), implementation and ongoing evaluation of habilitative service plans that are based on the person’s preferences, needs and interests related to vocation. Helps people to develop/maintain positive interpersonal relationships with their peers, with others in their families and with people in the community.

7. Assists people in developing their vocational skills and exploring career options.

9. Identify and secure suitable internship and volunteer opportunities for people served, based on choice, needs, preferences, and person’s abilities. Provides orientation and maintains relationships with past, current and prospective community contacts.

10. Actively participates at staff meetings by communicating observations concerning progress and reaction to various therapy programs.

11. Completes accurate documentation in Therap within required timeframes, based on the implementation of the habilitation plan.

12. Provides assistance with navigating the community up to and including providing transportation to people we support when necessary.

13. Completes DDP-2s, incident reports and other agency forms necessary for service delivery.

14. Provides ongoing communication within and across departments to ensure proper service delivery is provided and maintains a minimum of one monthly contact with other service providers of all individuals on caseload and documents these contacts in case notes.

15. Purchases appropriate program supplies with the approval of the Manager.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

WORKING CONDITIONS

1. Routinely works normal business hours. On occasion, evenings and weekends are required when the needs of the individual or the employer cannot be met during regular business hours.

2. Local travel required approximately 25% of the time. The remaining time is used providing training directly with individuals, developing internship and volunteer placements and completing necessary paperwork.

Requirements

1. Education and Experience

a. High School Diploma/GED

2. Skills, Knowledge, and Abilities

a. Knowledge of the local job market.

b. Basic knowledge of how to maintain/operate maintenance equipment.

c. Good oral and written communication skills

i. Must be able to read and write case notes as well as understand written directions.

d. Ability to manage multiple tasks and cope with stressful situations and deadlines.

e. Demonstrate the ability to exercise independent judgment and maintain concentration.

f. Create and maintain positive interaction with others.

g. Basic computer skills.

3. Certifications

a. CPR/First Aid certified.

b. SCIP-Core certification

4. Be authorized as an Agency Driver by

a. Holding and maintaining a valid NYS Driver’s License.

b. Maintaining an approved driving record according to Agency Policy and Procedure.

5. Verifications and screenings

a. OPWDD clearance for convictions of criminal offenses.

b. Clearance through NYS Central Child Abuse and Maltreatment Register.

d. Tuberculosis testing.

e. Approval by NYS Justice Center.

6. Physical, Medical and Agility Requirements

a. Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs as well as the ability to perform all required movements required by the SCIP-R curriculum.

b. Lifting up to fifty (50) pounds.

c. Staying awake and alert while on duty.
