Job summary

Towne Home Care in Middlesex, NJ is seeking qualified Direct Support Professionals to provide one-on-one supports in private residences and community settings under the ISP.

You will accompany individuals to appointments, assist with personal care, mobility, and daily tasks, and document progress for the ISP. Daytime shifts preferred; applicants must have DSP training and CPR/BLS.

Qualifications

• Completion of initial required DSP training modules through the College of Direct Supports.
• CPR/BLS certification.
• 1 Current PPD Test (Single Step PPD) or Chest X-Ray.
• Excellent verbal and written communication.
• Access to a registered vehicle, proof of valid auto insurance, and valid driver’s license.
• Must be comfortable driving client daily in own vehicle to various local outings and appointments.

Responsibilities

• Provide one-on-one supports in private residences and in the community per the ISP.
• Support development of ISP as part of the interdisciplinary team.
• Assist with domestic and household tasks.
• Assist with personal care activities.
• Assist with mobility and physical therapies/exercises.
• Provide companionship and basic emotional support.
• Complete logs linking activities to ISP goals.

Towne Home care is seeking qualified and experienced Direct Support Professionals around Middlesex - DSP who will continue our tradition of providing excellent individual - centered supportive care.

Applicants who are available to work daytime shifts are strongly preferred.

Responsibilities:

• Provide individual and community based supports to individuals in private residences and in the community on a one-on-one basis in accordance with the individuals ISP (Individualized Service Plan).
• Act as a member of each individuals interdisciplinary team to assist with the development of their ISP.
• Assist and support the individual to perform domestic and household tasks.
• Assist individuals in completing personal care activities.
• Assist individuals with mobility and physical therapies/exercises.
• Provide companionship and basic emotional or psychological support.
• Complete thorough logs outlining activities completed and connecting them to the goals of the ISP.

Skills:

• Proven working experience as DSP preferred
• Usage of a smartphone for case info and clocking in and out
• Familiarity with basic nutrition and personal hygiene standards
• Caring and compassionate personality
• Ability to assess changing situations and respond accordingly.

Requirements:
