Job summary

Axiom Care in Phoenix, AZ seeks a Patient Transportation Driver to coordinate patient movement among hospitals, clinics and our facilities. You will be the first representative patients meet, delivering compassionate service and ensuring safe, timely transport.

The role requires empathy, strong communication, and the ability to de-escalate crises while supporting recovery-oriented care. Valid AZ driver's license and clearance are essential.

Qualifications

• Must show empathy, compassion and professionalism with patients.
• Proficient computer literacy and basic office software.
• Ability to organize time and manage multiple tasks efficiently.
• Experience with diverse populations; Native American experience preferred.
• TB test current within the last 12 months and valid clearance.

Responsibilities

• Pick up patients from hospitals, police stations, jails and transport to Axiom facilities.
• Safely transport patients to offsite appointments as coordinated with clinical teams.
• Orient new patients to the facility and introduce the treatment team.
• De-escalate crisis situations as needed and provide support.
• Leverage recovery experience to encourage patient engagement in treatment.
• Ensure warm hand-off when transferring clients between facilities.
• Build relationships with referral partners and present professionally.
• Assist with patient intake and EMR onboarding as needed.
• Act as front-desk receptionist when required and direct visitors to staff.
• Promote CLAS standards to ensure culturally and linguistically appropriate care.
• Perform other duties as assigned by management.

Description

The Patient Transportation Driver is responsible for coordinating with intake, clinical, medical, and case management teams to connect patients with the services they require. The Patient Transportation Driver is often the first representative of Axiom Care that a patient will meet and must be able to conduct themselves with professionalism and show compassion for our patients.

DUTIES/RESPONSIBILITIES

• Pick up patients from hospitals, police stations, jails, other medical facilities, and Axiom referral partners, and transport them to the appropriate Axiom facility.
• Primary staff responsible for safely transporting patients to offsite appointments as coordinated with their clinical or nursing teams.
• Help orient new patients to Axiom facility – introduce patient to treatment team, give tour of facility, describe rules and schedules, etc.
• De-escalate crisis situations as needed.
• If/when appropriate, draw from personal recovery experience to encourage patients to actively seek recovery.
• Transport client's post-treatment to their next treatment setting and ensure warm hand-off with receiving facility, whether another Axiom facility or not.
• Outreach and build relationships with representatives of Axiom’s referral partners, presenting self professionally.
• Assist with patient intake process as needed, include property inventory, patient orientation, and EMR onboarding.
• Act as front-desk receptionist when needed by greeting visitors, answering incoming calls, and directing to facility staff as needed.
• Promotes inclusion through Culturally and Linguistically Appropriate Services (CLAS) Standards.
• Performs other duties as assigned by management.

Requirements

Required Skills/Abilities

• Experience in working with diverse populations and socio-economic backgrounds.

• Negative TB Test results within the past 12 months.
• Fingerprint Clearance Card upon hire or in process.

Education and Experience

• Minimum of a HS Diploma or equivalent is required
• 1-year behavioral health experience OR personal or professional experience with substance abuse treatment required.
• Peer Support Certified preferred with at least 1 year sobriety.
• Must have a valid AZ Driver's license with 39-month clear MVR
• CPR/First Aid certification required.

Physical Requirements:

• Physically able to drive a vehicle for prolonged periods of time.
• Must be able to lift and move luggage weighing up to 50 pounds.
