Job summary

OPCO Skilled Management is seeking a highly skilled Registered Nurse (RN) or Licensed Vocational Nurse (LVN) for the Charge Nurse (Double Weekend) role at Brentwood Place Three in Dallas, Texas. You will lead a Weekend Shift team delivering compassionate, patient-centered care in a skilled nursing and rehabilitation setting.

The role focuses on overseeing care plans, supervising nursing staff, administering medications, and educating patients and families while maintaining accurate documentation

Responsibilities

• Deliver high-quality patient care to a diverse range of residents, including those recovering from surgery, injury, or illness
• Work collaboratively with the rehabilitation/nursing team to develop and implement individualized care plans, ensuring optimal outcomes and improved quality of life
• Monitor and assess patient conditions, reporting any changes or concerns to the healthcare team
• Administer medications, treatments, and interventions as prescribed by physicians and in accordance with facility policies
• Educate patients and their families on disease management, medication use, and post-discharge instructions
• Maintain accurate and detailed records of patient care and progress
• Participate in quality improvement initiatives and staff development programs to ensure continuous growth and improvement
