Job summary

Continuum Therapy Partners is seeking a Director of Rehab to lead rehabilitation services across Saint Charles, MN locations. You will oversee planning, coordination, and delivery of PT/PTA programs and ensure licensure compliance for residents and facilities.

We offer an $8,000 sign-on bonus, full benefits, 401k with company match, licensure reimbursement, and generous PTO. This full-time daytime role places you at the center of patient care and team collaboration.

Qualifications

• Graduate of an accredited therapy program (PT/ PTA).
• Current state licensure.

Responsibilities

• Provide professional support services to rehabilitation center residents.
• Initiate referrals as suitable and complying with Therapy Department procedures.
• Plan, schedule, arrange, direct and oversee entire rehabilitation services to ensure optimum departmental operation.
• Establish program development objectives and conduct ongoing analysis of effectiveness.
• Develop and maintain relations between Continuum employees and facility staff and administration.

Director of Rehab Requirements & Responsibilities:

• Graduate of an accredited therapy program - Physical Therapist (PT), Physical Therapist Assistant (PTA)
• Current state licensure
• Provide professional support services to rehabilitation center residents.
