Director of Nursing/ Health and Services Director (Residential Facility)

Summary: The Director of Nursing is directly accountable for all resident care activities performed within the facility on a twenty-four-hour basis. He/she coordinates nursing service activities with facility administration, community agencies, physicians, and families. He/she assumes responsibility for nursing service compliance in accordance to facility policy and procedure as well as Federal Regulations and State Rules governing the facility.

Reports To: Administrator

The prerequisites below are representative (and not an exhaustive collection) of the knowledge, skill, and/or ability required to successfully perform this job. Professional Qualifications:
1. Is a graduate from a school of professional nursing.
2. Is currently licensed by the State of Indiana.
3. Is experienced or trained in nursing services administration, rehabilitation, or geriatric nursing.
4. Preferably, has one or more years of demonstrated ability in nursing administration and/or supervision in a health facility.
5. Must possess a current health certificate/examination required by State agencies.

2. Demonstrates leadership ability and capability to work effectively with all levels of personnel.
3. Maintains an interest and knowledge of current trends and new developments in nursing and health administration.

5. Ability to communicate verbally and in writing.
6. Should possess good managerial skills and a strong sense of responsibility.
7. Demonstrate a willingness to work with the realization that resident outcome is determined by the quality of nursing care provided.
8. Maintain good working relationships with professional medical persons, and have willingness to increase knowledge in nursing, through individual study, participation in training and formal courses/certifications when available.
9. Possess skills in the assessment of nursing care needs of the residents.
This position is a job classification/category for which the employee’s duties will cause the employee to have occupational exposure to blood or other potentially infectious material (i.e., body fluids).

Essential Job Functions:
1. Plan, develop, organize, implement, evaluate, direct, and manage the nursing services department.
2. Develop, maintain, and periodically update written nursing policies, procedures, manuals, objectives and philosophies, if necessary.
3. Implement plans of action to correct deficiencies.
4. Complete medical forms, reports, evaluations, studies, charts, etc.
5. Participate in surveys and inspections by government agencies.
6. Serve in executing administrative functions and decision-making in the absence of the facility’s Administrator.
7.Coordinate nursing services with other services/departments.
8. Direct the planning, implementing and evaluation of nursing care services for all residents on a 24-hour, 7 day a week basis.
9. Participate in the interviewing and screening of residents for admission to the facility.
10. Assure that each resident’s attending physician(s) and family or representative, are promptly notified of any significant change in the resident’s health status.
11. Recruit, interview, hire, counsel, evaluate, discipline, and terminate nursing services personnel.
12. Develop work assignments for nursing services personnel in keeping with specific nursing care needs of the residents.
13. Advise the facility Administrator as to the number and qualifications of nursing personnel required to meet the residents’ needs.
14. Assure that nursing services personnel are not required to perform any duties or functions outside of their scope of practice.
15. Delegate administrative authority, responsibility, and accountability to nursing services supervisory personnel as deemed necessary to perform assigned duties.
16. Maintain professional competence, as well as current licenses, through participation in continuing education programs, seminars, and training programs.
17. Maintain resident confidentiality and privacy.
18. Treat residents with kindness, dignity, and respect.
19. Know and comply with and ensure that all nursing services personnel know and comply with Residents’ Rights.
20. Monitor nursing services to ensure that Residents’ Rights and are honored.
21. Promptly report to the Administrator all resident complaints and grievances.
22. Assist in developing for each resident a preliminary assessment and written service plan that identifies the nursing and medical concern and/or needs of the resident.
23. Ensure that all nursing services personnel are aware of the service plan and availability for use in providing daily nursing services to the resident.
24. Monitor nurses’ notes and monitor residents to determine if the service plans, are being followed and if each residents’ needs are being met.
25. Participate in assessing, reviewing and revising service plans as required.
26. Develop and maintain rapport with all services to ensure that a team effort is achieved in developing service plans for all residents.
27. Recommend to the Administrator the equipment and supply needs of the nursing services department.
28. Attend and participate in various committees and meetings of the facility.
29. Assist in developing, implementing and maintaining safety standards, written aseptic and isolation techniques, programs for monitoring communicable and/or infectious diseases among residents and personnel and policies and procedures for reporting hazardous conditions or equipment.
30. Confirm that nursing services personnel follow established infection control procedures when isolation precautions become necessary.
31. Confirm that nursing services personnel understand and follow departmental policies and procedures, Universal Precautions, established safety rules and fire and disaster procedures.
32. Assist with developing and implementing methods for coordinating nursing services with other resident services.
33. Assist in planning, developing, implementing and maintaining resident discharge procedures and plans.
34. Confirm that all nursing services personnel have valid and current licenses or certifications and are performing duties in accordance with acceptable nursing standards.
35. Review personnel complaints and grievances and ensure that all personnel are treated consistently with policies and applicable laws.
36. Participate in planning, implementing, conducting and scheduling orientation, training and in-service educational activities for nursing service personnel.
37. Make periodic inspections of supplies and equipment to ensure that adequate levels are maintained and that equipment is available and working properly.
38. Confirm cleanliness of nursing services work areas, stations, preparation rooms, treatment areas and resident care rooms.
39. Other duties as assigned by the Administrator.

Physical, Sensory, and Communication Requirements:

1. Ability to read and write in English, ability to communicate with residents, families, personnel and support agencies and ability to remain calm under stress.
2. Bending, lifting, walking, grasping, fine hand coordination, ability to distinguish smells, tastes and temperatures, ability to discern colors, ability to hear and respond to pages.
