Job summary

Crosspoint Human Services is seeking a Domestic Violence and Shelter Programs Care Coordinator in Danville, IL to provide service coordination for homeless individuals or survivors of domestic violence. You will ensure safe, therapeutic, and empowering support as clients navigate critical life transitions.

The role includes completing assessments, coordinating care plans, and collaborating with an interdisciplinary team to promote safety and recovery.

Qualifications

• Bachelor's degree required in a relevant field.
• Two years work experience in the human services field is preferred.
• Excellent organizational and time management skills.
• Valid driver's license and proof of automobile insurance.

Responsibilities

• Facilitate individual and group support to promote growth, safety, and empowerment.
• Maintain accurate and timely service documentation.

Job description

Be the bridge to safety and stability for those in need. As a Domestic Violence and Shelter Programs Care Coordinator you will work directly to provide service coordination for individuals who are homeless or survivors of domestic violence. In this role, you will ensure individuals receive safe, therapeutic, and empowering support as they navigate critical life transitions.

Crosspoint Human Services offers comprehensive on the job 40 hour domestic violence training. .

Responsibilities and Duties:

Duties include but are not limited to:

• Completing comprehensive assessments and collaborating with individuals served to develop a care plan that is tailored to their unique needs and goals.
• Facilitating individual and group support that promotes growth, safety, and empowerment.
• Maintaining accurate and timely documentation of services provided.
• Participating as a member of the interdisciplinary team to ensure comprehensive coordination of care.

Job Requirements

• Bachelor's degree in social work, counseling, human services, psychology, or related field.
• Two years work experience in the human services field is preferred.
• Excellent organizational and time management skills.
• Valid driver's license and proof of automobile insurance
