Job summary

Qualifications
- Two years of experience working as a registered nurse with individuals with disabilities in home health, hospice, or long-term care.
- Must possess a valid Level One fingerprint clearance card to work with children and vulnerable adults.
- Possession of a current license to practice as a registered nurse in the State of Arizona.
Responsibilities
- Coordinates and monitors physical health for skilled nursing service needs.
- Conducts visits to develop relationships and familiarize with family dynamics.
- Conducts comprehensive assessments of physical, behavioral, and long-term care needs.
