Job summary

Montefiore Nyack Hospital seeks an experienced Clinical Operations Lead – Case Management. The role coordinates patient care progression, discharge planning, and CMA workflow, with accountability for throughput, length of stay, and timely transitions of care.

The position requires BSN, advanced nursing leadership, and strong collaboration with physicians, nursing, and interdisciplinary teams to optimize discharge readiness and patient flow. 8-hour days, day shift.

Qualifications

• BSN required; Master’s in nursing, healthcare administration, or related field preferred.
• Minimum 3–5 years of acute care nursing experience; 2–3 years in Case Management or care coordination.
• Experience with discharge planning, transitions of care, and interdisciplinary collaboration.

Responsibilities

• Drive patient progression and discharge efficiency through proactive management of clinical milestones.
• Oversee CMA workflow with real-time prioritization and task allocation.
• Collaborate with physicians, utilization review and revenue cycle to ensure timely progression of care.
• Lead discussions on care progression and discharge readiness; participate in hiring panels for CMA staff.
