Job summary

Montefiore Nyack Hospital, Nyack NY, seeks a Supervisor, Care Management to lead the hospital-wide Care Management Program, ensuring high-quality, cost-effective care through collaboration with physicians, care managers, patients and families, insurers, and the health care team.

You will direct length of stay decisions, oversee transitions of care, and work with the Director of Care Management on clinical initiatives and day-to-day management.

Qualifications

• Education: Graduate from an accredited School of Nursing or college. BSN Required. MSN or MPA Preferred.
• Experience: 2-3 years’ experience in hospital utilization review and/or managed care. 5 years’ experience in an acute care setting or equivalent. 1-3 years of management experience preferred.
• Training/Certification: Case Management certification preferred.
• Licensure: Current New York state RN license required.
• Knowledge of DOH, JCAHO and Peer Review Organization requirements. Knowledge of PC based systems to assist in data analysis. Effective written and verbal communication with medical and administrative staff. Strong interpersonal skills.

Responsible for supporting and coordinating hospital-wide Care Management Program to ensure the delivery of high quality, cost effective care through collaborative relations with physicians, care managers, patients/family, insurance carriers and other member of the health care team. Directs appropriate length of stay decisions based on knowledge of State, Federal and Third Party regulations to decrease LOS, perform transitions of care and initiate timely discharges. In addition to the day-to-day responsibilities, the Supervisor, Care Management will work in conjunction with the Director of Care Management on a variety of critical tasks including the management of one or more focused clinical initiatives.

B. Essential Job Qualification

1. Education: Graduate from an accredited School of Nursing or college. BSN Required. MSN or MPA Preferred.
2. Experience: 2-3 years’ experience in hospital utilization review and or managed care. 5 years’ experience in an acute care setting or equivalent experience. 1-3 of management experience preferred. Strong problem solving skills required.

Ability to communicate (written and verbal) effectively with medical and administrative staff in a positive, assertive, and professional manner. Self-motivated, self-directed, well organized with excellent interpersonal skills required. Demonstrated competency knowledge and ability to effectively care for the adult geriatric patient population.

1. General Contacts: Frequent communication with management, medical staff, department heads, home care, finance, support staff and board members. Contact with outside review organizations, insurance companies, nursing homes, assisted living/adult homes, rehabilitation facilities and regulatory agencies.
