Sinai Hospital, part of LifeBridge Health, is seeking a Discharge Support Specialist to coordinate post-acute referrals and discharge planning with our Care Management team. You will ensure timely notifications and documentation, collaborating with patients, families, and community providers to promote safe transitions of care.

Sinai Hospital's inpatient Care Management team provides discharge planning, transitional care coordination, complex care management, and utilization review through a multidisciplinary team of registered nurses, LPNs, licensed social workers, and administrative staff. While many functions are dictated by regulatory requirements, the core clinical focus is always on ensuring a safe and effective transition to the next appropriate level of care. Your work directly supports safe discharge planning, reduces barriers to care, and improves patient outcomes.

About the Role:

The Discharge Support Specialist supports the Care Management team by coordinating post-acute referrals, facilitating discharge planning activities, and ensuring patients receive required notifications and documentation. This role helps promote safe, timely transitions of care by collaborating with patients, families, care managers, and community providers.

Key Responsibilities:

• Support discharge logistics by arranging follow-up appointments, verifying patient information, coordinating post-discharge services, and providing administrative support to facilitate efficient patient discharges
• Ensure compliance with patient notification requirements by delivering discharge-related notices, obtaining required signatures, and maintaining accurate documentation within the medical record.
• Coordinate post-acute care referrals, track authorizations, communicate referral status, and coordinate delays or barriers to care transitions.

Requirements:

Education: High School/GED or Associate's Degree

Experience: 1-3 years; medical terminology preferred

Pay Range: $17.00 to $24.75 Based On Experience
