The Discharge Specialist provides case management services to patients, completing discharge plans for each patient's transition to the next level of care. They introduce programs and services designed to meet the needs of patients after discharge and facilitate discharge planning groups on each unit.

Responsibilities

A typical day for a Discharge Specialist at Hackensack Meridian Health includes:

  1. In conjunction with the treatment team, formulating discharge plans for patients on the first day of admission.
  2. Beginning the discharge planning process within 24 hours of assignment.
  3. Participating in the treatment planning process.
  4. Continuously updating the ACT database.
  5. Facilitating discharge planning groups weekly on assigned units.
  6. Demonstrating positive communication techniques with treatment team members to enhance patient care.
  7. Performing other duties and projects as assigned.
  8. Adhering to HMH organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills, and Abilities Required:
  1. High School diploma, GED, or equivalent programs.
  2. Minimum of 1 year relevant healthcare work experience.
  3. Excellent written and verbal communication skills.
  4. Proficient computer skills, including but not limited to Microsoft Office and Google Suite platforms.
Preferred:
  1. Bachelor's Degree in Social Work or related field.
