Qualifications

• LCSW, LSW, or MSW license required.
• Bachelor’s or Master’s degree in social work.
• Must have a valid driver’s license.
• Experience in post-acute/ skilled nursing preferred.

Responsabilités

• Assesses the psychosocial status of patients and communicates findings to the registered nurse.
• Carries out social evaluations and plans person-centered interventions.
• Maintains clinical documentation on all patients referred to social work.
• Coordinates referral services for residents and families regarding practical and environmental needs.
• Provides information to residents or families about community services.
• Serves as liaison between residents and community agencies.
• Participates in the development of the total plan of care and case conferences.
• Assists the physician and care team in understanding social and emotional factors.
• Supports discharge planning and post-discharge needs.
• Coordinates medication review to optimize dosing.
